Tryout Registration

Any photographs taken of your children and/or their artwork at Sally Studio is the property of Sally Studio and can be used in any marketing, advertisement or other use. Waiver: Parent or guardian hereby gives permission to my child to participate in classes and instruction at Sally International Design and/or Sally Studio. Parent and/or legal guardian agrees to hold harmless, release and covenant not to sue Sally Studio, Sally International Design, the owners, lessors lessees and/or operators of the premises where such classes are held, from any liability, losses, claims and damages due in whole or in part due to the claimed negligence of Sally International Design, Sally Studio or any of the above-named. I have read this release and I realize that I am giving up a right to sue and I do so voluntarily and without inducement or assurance of any kind and I intend this to be a complete and unconditional release of liability. Parent also acknowledges having read and understood Sally Studio policies.

A one-time, non-refundable fee of $25 is required for all new students who wants to sign up our School Year Semester classes. If your child enrolls after the tryout, the new student fee will be waived. Please complete and submit this form before attending the tryout class. Once we receive your form, we will confirm and notify you of the next available tryout date via email. Our tryout classes are held in group settings, typically once a month on weekend afternoons. If you are unable to attend Sunday afternoons, please contact us before registering to see if we have additional tryout dates.
